Over 60% of LLCHC's patients and families
currently have a substance
abuse and/or mental health diagnosis in addition to multiple chronic
medical concerns. Effective, comprehensive care requires a team of
multidisciplinary
providers and services. Medical care and behavioral health
care is integrated fully to achieve better health outcomes.
Our behavioral health services include clinical counseling,
psychiatric
assessments, medication management, and enabling services including
eligibility assistance, financial counseling and limited case
management.
LLCHC uses Patient Advocates to help assist our patients in
decreasing
barriers to care and increasing compliance. The advocates also train
lay
health volunteers to educate neighborhood residents - increasing
health
awareness and education and decreasing inappropriate ER use.

patient about 8 years
ago she was a prostitute living on and working the streets to feed
her crack habit.
She was emotionally, spiritually, and physically dead. Over the last
8 years she has
gotten sober and worked on recovery, she now has permanent housing,
